Presentation Summary: High incidence of opioid overdose among people recently released from prison highlights a need to support formerly-incarcerated individuals with substance use disorders (SUD) in transition back to community. CAI’s SUCCEED intervention (SUD as a Chronic Condition Engagement & Education) builds capacity among community organizations to provide training and supportive services to individuals in this transition. SUCCEED emphasizes that SUD is a chronic condition, manageable with both medications and skills (e.g., recognizing signs of relapse, co-creating a recovery plan). In this session, staff from CAI and Santa Maria Hostel in Houston will share promising results from SUCCEED in supporting returning individuals.
Learning Objectives:
At the completion of this activity, participants will:
Describe how CAI’s SUCCEED model aims to support individuals with histories of SUD and justice involvement in their transition back to their community after incarceration.
Describe how the intervention has promising results both at participating agencies and for the justice-involved clients they serve.
Discuss how SUCCEED meets the emerging need to support staff and CBOs in the care and services they provide to transitioning individuals to prevent relapse.
