Presentation Summary: A high rate of discharge against medical advice, patient-directed discharge, is seen in hospitalized patients with opioid use disorder. Despite being associated with a lower risk of patient-directed discharges, initiating opioid use disorder treatment amongst hospitalized patients with opioid use disorder has been shown to be underutilized and underdosed. Presented here is a case report utilizing telemedicine to link inpatient methadone initiation with dose adjustment guided by an Opioid Treatment Program provider to outpatient methadone maintenance therapy.
